Fundraiser Instructions

1. BE SAFE! Kids, don’t sell alone and sell only to people you know such as relatives, friends and neighbors. “Parents, take order form to work!”
2. PRE-COLLECT $14 on all items sold. Make checks payable to West Plains Soccer.
3. Order forms, when turned in must:

a)    Have seller’s name, phone number and team/coaches name.

b)    Be totaled at the bottom of each column with quantity sold. (not dollar amount)

c)    Be neat. NO lines drawn through mistakes. Use a new form if needed.
