· U-6, U-8, U-10, U-12, U-15, and Adult teams

· Female teams if enough teams sign up

· $40 Team Entry fee

· Form your own team or contact us to be assigned a team

· New game format – 3 v 3, no goalies. See westplainssoccer.com for tournament rules.

· Games start at 10:00 AM

· Trophies for first place team in each age division.

In consideration of my entry into the 3 vs. 3 West Plains Soccer Tournament, I, intending to be legally bound, do hereby for myself, executors and administrators waive, release, and forever discharge any and all rights and claims for damages, including any claims for loss, damages, or injury to my person or property arising out of my participation in this tournament, from the West Plains Soccer Association and their respective board members, officers, employees, agents, and representatives.

I, the undersigned agree to abide by tournament rules and do hereby authorize and consent to any first aid, medication, medical treatment or surgery deemed necessary in case of emergency.
